Health and Adult Social Care in Redbridge

 

In 2016, London Borough of Redbridge and NELFT joined together in partnership to provide integrated community health and adult social care services. We were amongst the first boroughs to do this and as a result, people who use our services are offered a seamless community service.

We operate from four local bases meaning that we can respond to the needs of our local community in ways that centralised services cannot. We also provide day services and extra care so that people can live as independently as possible in their community with choice and control over their everyday lives.

Our service continues to innovate and develop to respond to changing needs. We are transforming many of our services including those for learning disabilities and day services as well as the support we offer carers, people with dementia and those receiving care in their home.

About Health and Adult Social Care Services

Redbridge were rated as one of the highest performing councils for Adult Social Services across the whole country in the recent Inpower Survey.

We help adults with health and social care needs find support so they can live as independently as possible in their own homes. This includes older people, people with physical or learning disabilities, and mental health service users. Social workers have great opportunities to develop their practice in our diverse borough. About our service

Redbridge’s Health and Adult Social Care Service is an innovative partnership bringing together Health and Social Care services to provide integrated, sustainable, quality health and social care services for the residents of Redbridge. It is a unique partnership, which uses multi-disciplinary community teams approach to providing integrated care and support at a local level.

Managed by NELFT NHS Foundation Trust, through a Section 75 partnership agreement with London Borough of Redbridge Adult Social Services, the service started on 1st April 2016. This unique partnership delivers services at a local level from four bases around the borough, creating services which respond to the needs of our local communities.

Key design features include:

An enhanced ‘front door’ with a single point of access providing:

  • Comprehensive advice and signposting informed by good local knowledge;
  • Crisis and quick intervention where necessary; and
  • Initial well-being assessment delivered by skilled Wellbeing Officers.

Locality based multi-disciplinary team approach which ensures:

  • A clear pathway developed with stakeholders including people who use the service;
  • There is a focus on person centred holistic support planning to maintain independence by promoting health and wellbeing;
  • That team responsibility for delivering a service based is based on where a person lives not presenting needs;
  • A joint assessment approach, which covers both health and social care needs; and
  • A care coordination approach, which provides a single point of contact for people and their carers.

Our offer for employees is a culture that allows an integrated approach by being:

  • Respectful of the contribution of different specialisms bring, fostering a joint approach to problem solving;
  • Less bureaucratic, delivering holistic services designed around individual need rather than service need;
  • Encourages staff from different disciplines, to develop support plans which are person centred and focussed on independence, health and wellbeing; 
  • Supportive of positive risk taking;
  • Understanding that crisis intervention is planned and agreed with a view to creating long term independence and control rather than increased dependency; and

About NELFT

NELFT provides an extensive range of integrated community and mental health services for people living in the London boroughs of Barking and Dagenham, Havering, Redbridge and Waltham Forest, and community health services in south west Essex across Brentwood, Basildon and Thurrock. NELFT also provide an Emotional Health and Wellbeing Service (CAMHS) across the whole of Essex.

NELFT became a foundation trust in 2008. This means that working slightly differently from other NHS organisations. NELFT are still firmly part of the NHS, and are subject to the same standards and inspection frameworks as all other NHS trusts, but have greater freedom in how resources are used to improve the care for the people who using services. With an annual budget of £550 million and employing 6,000 staff, NELFT provide a service to a diverse population of 1.5 million people.

About Redbridge Adult Social Services

London Borough of Redbridge’s Adult Social Services provides comprehensive information, advice and support services to all Redbridge residents. We also provide and commission a range of social care services to Redbridge residents who meet the eligibility criteria for care.

We employ 520 members of staff and have an annual net budget of £57 million. Approximately 70% of the budget is spent on commissioned services from other organisations, but Redbridge has retained a small number of in-house services with Day Opportunity services and three extra care sheltered housing.

Our model

Redbridge Council and NELFT are committed to ensuring that the care and support delivered is designed to help people achieve outcomes that matter to them.  We are committed to helping people to live as independently as possible with choice and control over their lives.  We aim to achieve high quality, person-centred services that respect people’s dignity, rights and choices.

The models we use are strength based people centred. Following a successful pilot of the Three Conversation Model, we have fully adopted this approach, now called ‘People Matter’. This has resulted in a focus on early intervention and prevention to support people who are over the age of 18.

We have conversations with people to find out what is really important to them, what they would like to achieve and how to connect them to family and community support networks so they can best maintain their independence.

The People Matter model builds on the strength based approach as outlined in the Care Act which aims to support people to retain their independence, health and wellbeing for as long as possible.

  1. Conversation 1 supports people to best use their own strengths and connects them to community resources.
  2. Conversation 2 is about working with people in crisis intensively and dynamically to make an immediate and positive difference, sticking to them like glue for short period of time.

Conversation 3 supports people who may have longer term social care needs, focuses on what a good life means for them, whilst promoting identified strengths and community assets.
