Young people who receive a service from the 16+ Team will have
contact details for their own allocated worker.
To access this service:
Young people become 'looked after' having been subject to an
assessment of their needs. Looked after children and care
leavers receive aftercare service. (Once they fulfil the
statutory criteria set out in the Children (Leaving Care) Act
2000. Services to care leavers who meet the statutory
criteria ordinarily cease at 21 yeas, however care leavers who are
in continued further education after their 21st birthday can
continue to receive a service up to the age of 24 years, intended
to maintain support services through to the completion of their
further education course.
